Stable conditions, structured guidance, fewer escalations

Stable conditions deserve focused review, not full referrals

Endocrine conditions — particularly the stable ones — are well-suited to eConsult workflows. A patient with controlled hypothyroidism whose TSH has drifted slightly out of range does not need a six-week wait for an in-person endocrinology visit. They need focused specialist guidance on the medication adjustment.

Not every endocrine question requires a specialist visit. Many require specialist judgment.

The non-urgent question pattern

The clearest indicator that an eConsult is appropriate is the nature of the clinical question itself:

  1. Is the patient stable?
  2. Is the question focused on a specific decision rather than open-ended evaluation?
  3. Does the primary care team have the data needed to act on guidance?

When the answer to all three is yes, an eConsult typically delivers more value than a traditional referral.

What 'managing within scope' means

The most important outcome of well-structured endocrine consults is that primary care clinicians can confidently manage stable conditions within their existing scope of practice. Specialist input is reserved for the cases that genuinely require it.

Outcomes

Health systems implementing structured endocrine eConsults report a meaningful reduction in non-urgent specialty referrals — with no degradation in clinical outcomes.
